Camp Related Resources
Here are some forms and other documents related to Camp Crooked Creek.

  • Campership Application - The Lincoln Heritage Council has established a fund to help Scouts in need. Available only for Lincoln Heritage Council troops that are attending Camp Crooked Creek. [PDF]

Medical Forms
Medical forms are required by the Boy Scouts of America just incase something happens. All members should have at least a Class I medical history on file (this is good for anything under 72 hours). However, if you plan on doing anything over 72 hours, then look at Class II and Class III forms (both require physical examinations).

  • Class I and II (On same form, BSA #: 34414) - Class 1 Annual, Class 2 every 3 years. Good for youth and adults under 40 for activities. Can't be used for Adventure Bases or Jamborees (use form 34412A). [PDF]
  • Class III (BSA #: 34412A) - Annual; For all Class 3 activities AND all adults over age 40. [PDF]

Permits
Tour permits are REQUIRED for any excursion that your unit may go on. Select the one that's for you from below and be sure to allow enough time for your Council Service Center to process them!

  • Local Tour Permit (BSA #: 34426) - Good for domestic trips only under 500 miles away. [PDF]
  • National Tour Permit (BSA #: 4419) - Required for any trip (domestic or international) over 500 miles away. [PDF]
